Facility Use Form - General Information
Use this form to reserve space for events or groups at St Pete First United Methodist Church (including Sunday event/info tables in the Fellowship Hall). Please include 2 choices for room location, as your first choice may not be available. All events and groups must have approval before they can be scheduled. All audio/visual and food service needs must be approved by the Facilities Manager.

If there is a red "*" beside the question, you must answer it. If not, and the question does not apply to your event, you may skip the question. You will receive an email listing your responses. If you do not receive this email, please contact Kendra.
