Panther Kids Payroll Deduction 2023-24
Midway ISD Payroll Deduction Authorization

I hereby authorize MIDWAY ISD to make payroll deductions from my paycheck for the purpose of paying MONTHLY TUITION for PANTHER KIDS AFTER-SCHOOL PROGRAM.  It will remain in effect until: (1) the end of the 2023-2024 school year (final payment to be made in MAY 2024); or (2) my children are no longer in PANTHER KIDS by submitting a withdrawal form.


MISD Full Time Employees:  Elementary Per child - $95.00 per month, due Sept.-May; $855/year divided into 9 equal monthly payments.  Middle School Per child - $65.00 per month, due Sept.-May; $585/year divided into 9 equal monthly payments.  School days in August and the month of September tuition are covered by the September payroll deduction.
