2. Enter the information for each participant. If you are registering as a non-playing sponsor only, you may skip this section.
3. Enter the information related to your sponsorship. If you are registering as a player only, you may skip this section.

All playing participants will be required to pay by check or credit card prior to the event. Sponsor payments must be received in full before receiving advertising benefits. The primary contact person indicated on this form will be contacted to arrange payment. Please note that the discount for team registrations is only honored if registration and payment is conducted in one transaction (as opposed to four separate transactions, one for each team member).
