Student Request - Testing Center Accommodated Exam 
Welcome, students!  This form is for current AQ students eligible for testing accommodations.  Please complete this form if you would like to request an exam appointment in our testing center.

******REMINDER: Requests for all accommodated testing appointments should be submitted 7 days in advance (14 days for finals) to allow for proper planning and communication with your Professor.******

STEP 1 - DISSCUSS WITH YOUR PROFESSORS:
Before submitting a request with Accessibility Services for the testing center, students should talk with their professor about their testing accommodations.  Some may be well-met in the classroom environment or through collaboration with your professor (for example, 50% extended time). It is always preferable for students to test near their classroom so that they can ask questions during the exam and hear important updates.  

STEP 2 - SUBMIT YOUR REQUEST USING THIS FORM AT LEAST 7 DAYS BEFORE YOUR EXAM (14 DAYS BEFORE FINAL EXAMS):
All requests for exam appointments in the testing center should be made using this form and in accordance with this timeframe.

STEP 3 - NOTIFY YOUR PROFESSOR:
Upon each request, you must follow-up with your professor about your request.  Your professor will also need to submit a "Faculty Form for Accommodated Testing" and follow the appropriate steps to ensure your exam is ready for you on exam day!

A google calendar invitation will be sent to you and your professor with your testing center appointment details once the office has received both the "student" & "faculty" request forms.    

STEP 4- EXAM DAY:
Please arrive on time and prepared for your testing appointment. The Accessibility Services office is located in 103C Lower WEGE.  Please do not bring any non-essential items - only testing materials are allowed in the testing center.  Please be advised that there may be additional students in the testing center.
