Please read this entire message so you are not doing extra work by submitting forms that have already been completed. This is a reminder that sign-ups for Track & Field end this week. If you have 5th-8th grader interested in participating in Track & Field they need to be signed up by Friday, April 7th.

If you have already played a sport for the school in the fall or winter, you do not need to complete Permission to Participate Waiver, Parent/Athlete Concussion Information Sheet and Student Health Questionnaire. These only need to be completed once per school year

Nottingham families will also receive Google Forms related to the following Student/Athlete forms required to participate in school athletics. These additional sports forms include: (1) Permission to Participate Waiver Form, (2) Parent/Athlete Concussion Information Sheet, (3) Student Health Questionnaire From. These are required at the start of each school year (and/or prior to any participation in school sports) and are required to be completed prior to any participation in school sports....including tryouts. Please remember that if your child competed in fall or winter sports for the school.....you do not need to complete them a second time.
REMEMBER THAT YOU WILL ALSO NEED TO OBTAIN A CURRENT SPORTS PHYSICAL FOR YOUR STUDENT/ATHLETE PRIOR TO ANY TRYOUTS OR PRESEASON PRACTICES. If you have a recent sports physicals that was dated and obtained more recently - dated from May 31, 2022 or later - you should be all set to begin. I will notify those who have expired physicals as soon as sign-ups are submitted. Any copies of Sports Physicals should be forwarded to Stasia Cardillo, School Nurse.
