PERMISSION SLIP: GOLF
I approve of my son, details below, representing PNBHS at the Golf being held at Manawatu Golf Club on 1 May 2024.
I understand that:

The
team members will travel to MGC privately.

I
am required to pay $50 by 5.00 pm on 29 April 2024 for my
son to participate in this activity and that I need to contact the finance
office if I am unable to meet this deadline.

My
son will be subject to the Fundamental and Basic school rules during the entire
trip.
·
My
son should take part in such activities and such necessary duties as may be
required by Mr Smith.
·
I
need to notify the Main Office prior to departure of any changes in the medical
conditions of my son.
·
I
authorise the obtaining of medical assistance, if in the opinion of Mr Smith, such action is necessary.
·
I
need to notify Mr Smith prior to departure of any specific
circumstances that will require his consideration during this activity.
