As part of the internship course requirements, students are evaluated on their performance by their employer supervisor. This form should be completed by the individual that has the closest supervision of the work assignments completed by the student intern. Once completed, the evaluation is submitted directly to the Internship Program Director for review.

2.) We recommend to schedule an exit review with your intern in order to provide feedback on their overall performance.
