CONSENT AGREEMENT:
The purpose of this research project is to learn more about Post Trail Depression, inform hikers about the subject, and inform a book that I am writing. This is a research project being conducted by me, Rachel Meltzer.

Your participation in this research is voluntary. You may choose not to participate. If you decide to participate in this research survey, you may withdraw at any time. If you decide not to participate in this study or if you withdraw from participating at any time, you will not be penalized.

The procedure involves filling an online survey that will take approximately 15 minutes. Your responses will be confidential unless you fill out the contact sheet and give your consent for use. Use includes quotes about your feelings and experience (credited or anonymously, you may choose) and anonymous aggregate statistics. I will be gathering testimonials that I would like to credit to trail names if you are willing. There is a section where you can choose to participate in that. The Mental Health Assessment portion of the survey will remain anonymous no matter what.

I will keep your contact information confidential. All data is stored in a password protected electronic format. The results of this study will be used for writing purposes only but may be shared with an editor.
