By typing my name below, I certify that all the information I have provided is complete and accurate. I understand that incomplete or inaccurate information may impact my scholarship award. I understand that all the information in this application will be kept strictly confidential and only used to determine my eligibility for a scholarship.
If I am awarded a scholarship it will be for $455, the cost of the program tuition. All other expenses to attend the retreat are my personal responsibility including any additional payments required to Esalen including housing; I will arrange all further and necessary payments directly with Esalen.
