Please fill out the following information for your pet's grooming appointment prior to bringing them in. If you have any questions, please call us at 918-481-1693. THIS FORM IS REQUIRED SO OUR GROOMER HAS INSTRUCTIONS TO GROOM YOUR PET. A NEW FORM WILL BE REQUIRED FOR EVERY APPOINTMENT.

Please understand that we do not have a guaranteed pick-up time for your pet from grooming. The pickup time will be determined by how many dogs our groomer has, how big they are, and how much work is needed with each dog. Please initial stating you understand we CANNOT guarantee the time that your pet will be ready for pickup. *
