Waiver and Release of Liability ~ Lift Her Up Hockey is dedicated to lifting players up and providing a safe and fun place for new beginners and all levels of hockey players. Any player found making inaccurate statements about their level of play during registration that results in intentional injury to another player will be ejected from the League/Tournament without a refund. *

I hereby release, discharge relinquish, give-up, forego, waive and otherwise completely exonerate Lift Her Up Hockey, the employees, players, family, agents, assistants, officials, representatives or others acting on behalf of Lift Her Up Hockey, and any Arena facilities being used by Lift Her Up Hockey, for any liability, responsibility, culpability, or other basis upon which they may otherwise be liable for any injury, illness, disability, concussions, theft, incapacitation or other physical ailments which might arise in connection with Lift Her Up Hockey activities.

I hereby state that I am in good health and knowingly accept any health problems I may have, either known or unknown and recognize all risks and responsibilities inherent in the sport of ice hockey. I willingly and knowingly accept such risks in consideration of my participation in any Lift Her Up Hockey Season/Game(s)/ Tournaments.

I understand that Certified First Aid individual(s) will be available during games whenever possible but there is no guarantee of this.

I hereby agree to assume all risk for illness, injury, and damage to myself caused by any defect on the ice surface, in the equipment, or operation of any equipment used in connection with my attendance at the Lift Her Up Hockey Season/Game(s)/Tournaments and further specifically assume the risk of injury resulting from any personal equipment which I use or neglect to use including but not limited to: helmet, face mask, mouth guard, hockey skates, padding, sticks, or any other equipment used or not used during my participation in the Lift Her Up Hockey Season/Game(s)/Tournaments. I specifically assume any risks, which may arise in any way out of my consumption of alcohol or like products before or after each game and any social function related to Lift Her Up Hockey.

This instrument is signed by me (electronically or otherwise) voluntarily and I will hold all parties mentioned above and each of them free and clear from all responsibility by any reason of my being upon the property (inclusive of the arena parking lot, and surrounding perimeter) participating, practicing, or otherwise being involved in the Lift Her Up Hockey Season/Game(s)/Tournaments and will hold each of the parties mentioned above harmless from any court costs, attorney’s fees, or other expenses whatsoever caused by any suit or injury for damages brought by me or by one on my behalf.

All photographs acquired at above said Season/Game(s)/Tournaments become the property of Lift Her Up Hockey and may be used for promotional brochures, website viewing, and various forms of social media.

For Goalies, I am over the age of 18, have read and voluntarily agree to the terms of this release and specific assumption of risk. For Player's, I am over the age of 25, have read and voluntarily agree to the terms of this release and specific assumption of risk.

All participants are to advise Lift Her Up Hockey at time of registration via email to letstalk@liftheruphockey.ca of any known medical conditions that increase the risk to themselves or other participants. If medical conditions arise during the season that increase the risk to themselves or others, the player will notify Lift Her Up Hockey in writing via email to letstalk@liftheruphockey.ca of said conditions immediately.

I understand that a full team is equal to 9 players and 1 Goalie - in the event I'm registering as a team and the team does not meet these requirements an individual registrant may be placed on your team to fill it by Lift Her Up Hockey Admin

Disclaimer *

This league/season/tournament is geared towards beginner and rookie players therefore, I understand that if my level of play exceeds the beginner or rookie level, it will not result in intentional injury to other players. Furthermore, I acknowledge that Lift Her Up Hockey can at any time throughout the tournament or season eject me or my whole team for behaviour that does not align with this document in its entirety.

Refunds will not be given under the following circumstances. *

• Any injuries that occur on or off the rink or on your personal time which result in your season being cut short will not receive a refund.

• Once your first game has been played you will not receive a refund.

Refunds will be given for *

• If you have changed your mind and have not played your first game; your full registration amount, less an administrative penalty of $50.00 will be refunded. The only exception to this is in the event of an injury that occurs prior to the season or tournament, where a doctor’s note has been provided, your full registration will be refunded without the administrative penalty.

• If you have been asked to leave the game/league/tournament for any reason, you will be refunded a portion of your registration fee in the amount of $15.00 per game, for the remaining un-played games.

• If the season is cancelled prior to starting, your full registration will be refunded without an administrative penalty.

• If the season is postponed for any reason, makeup games will be given in lieu of the postponement. If a makeup game cannot be scheduled due to ice availability a refund of $15.00 will be e-transferred back to you.

Jerseys *

Jersey’s will be provided by the league. Your jersey will be collected, washed, and redistributed each week. If you have sensitivities such as allergies to detergents, please make this known in writing to the league in advance of your first game by emailing letstalk@liftheruphockey.ca • If you intentionally damage or fail to return your jersey by the end of the season or tournament you will be billed for a replacement at the current cost to replace.

Lift Her Up Hockey uses an attendance application that can be used on the Apple/Android/PC platforms. You are required to update your contact and emergency contact information in the app prior to your first game after you have received your invitation to the app. Please attach a photo of yourself so that it is easier for everyone to recognize you.

Lift Her Up Hockey will make every effort to connect teams with Goalies when possible, however, we do not guarantee one will be available.

Rules of the Game *

Once your registration is confirmed you will be emailed a pdf of the rules of the game. You will be required to sign this electronically (instructions will be provided)
