Medical Form Reminder
Students are required to have the following forms completed before attending any trip with Campus Life

1. A Trip Permission Slip (which is the form you are currently completing),
2. The 2021-2022 Campus Life Medical Form, and
3. Payment through TicketTailor Website

In addition to the previously mentioned forms, students attending the Smokies 2022 trip will require a waiver completed for our White Water Rafting adventure. This waiver must be printed and brought to check-in.
