By signing electronically per the Handbook Link, both the cheerleader and parent/guardian acknowledge that they have read and understand the following as outlined:

Both parties understand that if chosen as a member of any cheer squad, a commitment is  being made to that squad. It is understood that if the member quits or is removed from the  squad for any reason after squads have been announced, any monies paid in or fundraised  are non-refundable and the member will not be allowed to try out for or participate with any  spirit or competitive squad for one year from the date the cheerleader quit or was removed.  Spirit squads include all cheer and pom teams.

I understand that if selected for a  Bartlesville Cheer Squad, I will be expected to abide by these guidelines, as well as any other  rules designated by the coach or administrators. I understand that membership on any cheer  squad is both an honor and extremely hard work and I will make every effort to be a  successful member of the team by supporting my coaches, fellow squad members, and the  program throughout the year. I further understand that Bartlesville Cheerleaders are held to  the highest standards possible, and if I am selected, I will respect and honor the privilege  given to me to represent Bartlesville in the best manner possible.

I hereby authorize my above stated child to try out for cheer at  Bartlesville Public Schools. I understand that all decisions of squad placement made by the  coaches or coaches’ designees are final and each decision is made to maximize the potential  

for the success of each squad. If my child is chosen for the position of cheerleader, I  understand that they are obligated to serve in that capacity from the day of received tryout  results until the first day of tryouts for the next year. I have read the entire Bartlesville Cheer  Tryout Packet. I understand that these guidelines, as well as any others outlined by the  coaches or administrators, must be followed throughout the year and understand that  violation of any of these rules may lead to the prospective cheerleader not being allowed to  complete the tryout process, not being placed on a cheer squad, loss of points in grade or  accountability policy, benching or suspension, or permanent dismissal from the squad.

I am aware practicing and participating in any sport can be dangerous activity involving many risks of injury.  I do understand that dangers and risks of practicing any sport include, but are not limited to, death, serious neck and spinal injuries which may result in complete or partial paralysis, brain damage, serious injury to virtually all internal organs, other aspects of the body, general health and well-being.  I understand that the dangers and risks of practicing and participating in any sport may result not only in serious injury, but in a serious impairment of future abilities to earn a living, to engage in other business, social and recreational activities, and generally to enjoy life.  

Because of the dangers of participating in the above interscholastic athletic activity, I recognize the importance of following coaches' instructions regarding techniques, training and other rules, and agree to obey such instructions. 

In consideration of the Bartlesville Public School District (hereinafter "BPS") permitting me to engage in all activities related to the sport, including, but not limited to, trying out, practicing or participating in that sport, I hereby voluntarily assume risk of accident, injury or damage to person or property.  Furthermore, I voluntarily release and discharge the BPS, its employees, agents, representatives, coaches and volunteers from, without limitation, any and all action, cause of action, claims, demands, damages, costs, expenses, compensation, and/or suits at law or in equity, on account of or relating to any act or omission by the BPS, employees, agents, representatives, coaches or volunteers.  I also agree to defend, indemnify and save the BPS harmless from and against any and all liability, actions, causes of action, debts, claims, demands or suits at law, or in equity of kind and nature whatsoever which may arise, directly or indirectly, by or in connection with participation in any activity.  The terms hereof shall serve as a release for heirs, estate, executor, administrator, and assignees.
