USA Hockey requires all youth sled hockey games/leagues/tournaments to be conducted with players who are 17 years of age or younger as of December 31st of the current playing season. This USAH Age Waiver Form should be used to determine whether a player, who is over the age of 17 years of age as of December 31st of the current playing season, is eligible to participate with a youth team.
Age Guidelines & Age Waiver:
The youth division is for players 17 years of age and under as of 12/31 of the current playing season.
Players that are older than 17 years of age as of 12/31 of the current playing season and have participated in any USA Hockey sanctioned adult sled game(s) and players older than 18 years of age as of 12/31 of the current playing season, may be allowed to play in the youth division if a waiver request is submitted and approved by the waiver committee.
The purpose of this waiver is to allow players with more severe disabilities or players new to the game with a lower skill level to play in the youth division where it is less physical.
The US Women’s Development Sled Hockey Team & the US Men's Development Sled Hockey Team are considered adult teams. Any players who are 17 years or younger as of December 31st and have played on the either US Development Sled Hockey Teams are not allowed to play on their youth team in sanctioned games and tournaments without a waiver.
A team manager, coach, or player must complete this waiver request form along with a video of the player showing their on-ice skill level. Video should be sent to
shw@usahockey.org within three days of waiver request submission. Waivers requests will not be reviewed until video has been received.
Waiver requests must be submitted and approved prior to any participation in play.
The Age Waiver Committee will review age waiver requests starting September 1st through August for the respective season. Waiver Requests are reviewed on the Monday of each week, and a decision or follow up will be made by the end of that week.
