Students and Parents must complete this Google form, verifying that they have watched the Hilltop prerecorded virtual mandatory College Credit Plus Meeting for the 2021-2022 school year. By completing this form, both student and parent are stating that they attended Hilltop's mandatory Virtual College Credit Plus meeting, in order to be eligible to participate in the CCP program. They are also stating that they understand that failure to attend this meeting will make the student ineligible to take college courses the following school year.

Having been informed by local school officials, which includes the counselor and/or principal, and having received appropriate and adequate counseling regarding the College Credit Plus (CCP) program as called for by law, we the under-signed, choose to do the following *

Option A: I will take college coursework for college credit only. Under this option I am responsible for the expenses.

Option B: I will take college course work for college credit and high school credit to fulfill Hilltop graduation requirements. Under this option, the Millcreek-West Unity Local school District is responsible for the expenses.

Option C: We will not be participating in the CCP program for the 2021-2022 school year
