Acknowledgement of Risk Form - under 18's
This form is to be completed by a parent/legal carer of all participating children aged under 18, before the activity takes place.  If you are not legally responsible for any of the children in your group, please copy the link provided and ask the parent/carer of any child(ren) for whom you are not responsible, to complete this form for their own child(ren).

We would like you to read the following carefully:

It may affect your safety and the safety of others attending the activity, once you have read the information below, we would like you to please check the box as an indication that you have read and understood the content, and agree to proceed with the activity you would like to book with Anglesey Adventures (activity providers based on Anglesey Outdoors).

Adventure Disclaimer Notice:  Outdoor activities can involve physical and demanding sports, which obviously have inherent hazards associated with them. Whilst Anglesey Adventures take all necessary precautions to try and ensure the safety of all participants and provide you with the level of care and assurances of safety appropriate to these activities, unfortunately accidents will occur in consequence and you should be aware that certain inherent risks remain, which are integral to the activity and which cannot be eliminated without destroying their unique character. Each participant should familiarise themselves with the hazards and try to minimise these as much as possible by complying with Anglesey Adventures’ risk management guidelines. The owners accept no responsibility for any loss, injury or illness resulting from any persons’ involvement in outdoor activities. Furthermore, it is understood and agreed that individuals participate at their own risk.

Checking the box below on this Acknowledgement of Risk Form in no way compromises your legal rights, nor does it release Anglesey Adventures Ltd from any of its obligations towards you.  It is merely to make sure you are aware of what you are letting your child in for!

Amongst other things, some of these risks can contribute to:  The loss or damage of your child's personal clothing or equipment, feelings of discomfort, fear and apprehension, or even accidental injury, illness or trauma which, in extreme but thankfully rare cases, can be very serious.

The level of real (as opposed to apparent) risk associated with the activities of Anglesey Adventures is very low.  However, the type of risks may be something with which your child is not familiar.  The level of risk is generally no greater than that associated with, for example, the normal play of an average child, or that associated with a normal adult undertaking recreational activities appropriate to their ability, fitness, age etc.

Depending on which course, programme or activity your child is undertaking, they may be involved with and encounter any of the following:

Rough or rugged terrain – this may be on mountain or moorland, forestry or rough cultivated land etc.  If your child is only used to footpaths, tarmac surfaces and gentle inclines, this may come as a bit of a surprise by putting greater strain on joints and muscles than they are used to.

Physical effort – All our activities involve physical effort that at times may be more than your child is used to.  This may involve stamina or physical strength.  If your child has any medical conditions or injuries, past or present, which may make them ill during some activities, we advise you to make us aware of them prior to them commencing the activity.  This should not prevent them from taking part as we can almost always accommodate all participants, provided we are aware of the condition.

Water activities – All normal and appropriate precautions will be taken; such as the use of buoyancy aids.  However, some situations may become stressful for some people.  If your child has an aversion to water, you should inform Anglesey Adventures beforehand, although the ability to swim is NOT a requirement.

Environmental risks and hazards – These may include factors such as lightening, floods or rock-fall, although more common risks and hazards would be rapid and significant changes in the weather, or unstable or slippery conditions underfoot.  Slips and trips tend to be the most common type of accident throughout society and participating in our activities is no exception.  We will endeavour to ensure that the consequences of such a slip are not serious, but your child should be aware that the likelihood of falling over, or slipping is likely to be greater than they are used to.  Moreover, given the nature of the environment in which these may occur, the situation can compound giving rise to a more serious incident.

Infectious disease risks, including Covid-19 and its variants - Employees and visitors may be at risk of infection, or of spreading infection,  Anglesey Adventures have systems in place that assess the risk of and prevent, detect and control the risk of infection.  If you have symptoms of covid-19 or have recently been in contact with someone who has symptoms (within 14 days), Anglesey Adventures must be made aware and, if recent, you should not attend the activity, nor should any of the group you have been in contact with.  Staff at Anglesey Adventures are required to complete and submit a medical declaration form within 24 hours of an activity taking place; should they have symptoms an alternative instructor will be allocated.

Anglesey Adventures has clear obligations and responsibilities, which we take very seriously.  However, we will be expecting participants to contribute to their own and each other’s safety by following the instructions given, from time to time, by our staff.

If you are not comfortable with any of these, you should let that be known to us at the time of your arrival with us and preferably also in advance.  Alternatively, you may decide to withdraw your child from the activity; in this event your course fee will be refunded.
