I give permission for my child named above to walk to and from school without adult supervision. I understand that I am responsible for their safety and will ensure suitable childcare provision is at their destination. I will contact the school immediately should there be any changes to this agreement and will make instructions clear to a member of staff. I also understand that if the school have any concerns they will not allow my child to walk home alone and I will need to collect my child from the school. I also agree for my child to wear suitable safety equipment when using a bike or scooter. Failure to do this could result in parents being asked to collect their children, for the safety of the St Mary's children.
