I understand that the library will contact me, via phone or email, about two weeks  before I am scheduled to display my collection. I understand that I must respond to this contact to confirm the reservation. I understand that if I do not respond to confirm the reservation, I forfeit that reservation. *
I understand that my collection will be locked in the display case. I do not hold the library responsible for damage or theft. *
I understand that I am responsible for setting up my collection as early in the month as possible. I understand that if my collection has not arrived on or before the 10th, I forfeit the reservation. *
I understand that I must pick up my collection no later than the last day of the month. I understand that if I do not, the collection will be boxed up and stored in an unsecure staff area, and I will not hold the Library responsible for any damage or theft that may occur as a result. *
I understand that my collection must not contain items involving weapons or nudity. *
I authorize the library to post my first name and last initial on the display case, along with the name of the collection, as I have stated above.  *
I understand that the Library will not supply materials. I may bring push pins to attach items or decorations to the bulletin board background.  *
These items are not for sale to the public. *
I have carefully read this waiver on this form in its entirety and I understand that in lieu of signature, typing my name in this box is required in order for me to participate in Library District programs.
