This form is also the concent for all trips listed in the packet's schedule. This form will only be viewed by myself and administrators. Will you be renting an instrument from the school? If so, you will need to fill out the instrument use contract form found in the packet and bring it with your payment to the Sunday of band camp, July 25th. Percussion and Guard are assumed renters and will need to fill out the form and pay the cooresponding fees. *
My child and I agree to do our best to communicate and be active members of the Memphis Music Dep. following it's rules and procedures to the best of our abilities. This acts as my permission for my child to attend the trips stated in the Marching Band packet. *
