環工所網路使用申請登記表 MAC Address Application Form

注意：
1.  個人用：每人至多登記3筆資料(含先前已登記數)，如有超過，請在備註欄說明
         公用：務必在備註欄說明用途才可申請
2.  正確MAC卡號查詢請參見教學：https://reurl.cc/gzlV47，務必查看，尤其是蘋果產品
3.  網路問題回報：如果申請完仍然無法使用網路，請填寫表單(https://reurl.cc/gvznaL)回報，或到所辦洽詢網管。同一設備同一教室不要重複申請網路，不會有任何處裡
4. 此申請不會即時通過。如需急用，請告知陳姐直接請網管盡快設定。

1. For personal usage, principally, a person can register at most 3 devices. If exceed, please explain in the Remarks column.
    For public usage, please explain in the Remarks column.
2. How to find the MAC Address of your device: https://reurl.cc/qdVgR0 (Please check it out, especially for Apple products, please turn off Random MAC address)
3. Any problem for the applications or the internet, please report with the form(https://reurl.cc/gvznaL), or get the webmaster in the office for help. Please don't submit applications with 100% same data which will be ignored.
4. The application will not be passed immediately. If you need it in a urgent, please tell Miss Chen.
