This form is for you to express interest in being part of 2021 HAW Capital League Spitfires.

By submitting this form, the player nominated (and if under 18, their parent/carer) agrees to abide by the Hockey Australia Code of Conduct which is part of your Hockey Victoria registration.  You are also committing to be a financial member with Hockey Victoria in 2021.  

For all players under 18, all emails will be sent to the parent/carer, copy to the player.

This form must be completed and submitted before training or taking the field for Spitifres. Any issues please contact the HAW office.
