Please complete this form to provide us with information on your club and the project or event that you would like our clubs to collaborate on.

The information on this form will be forwarded to the appropriate committee in our club for consideration. However, due to the large number of collaboration requests that our club receives, please understand that we cannot guarantee that our club will proceed with the collaboration.

If a committee decides to proceed with the collaboration, you will be contacted directly via the email address that you provide in the form below.
