Outdoor Event and Field Trip Request Form
THIS REQUEST FORM IS ONLY REQUIRED FOR OVERNIGHT FIELD TRIPS.

Please submit the overnight Field Trip  Form at least five (5) days before the board agenda item.  You must still contact the Transportation Department Dispatcher Ann Marie Sesody (annmarie.sesody@fmsd.org) to request/coordinate any field trip bus drop off and pick up.  Don't hesitate to contact Lou Anne Castillo (louanne.castillo@fmsd.org) or Joni Policy (joni.policy@fmsd.org), if you have any questions or concerns.
