Band Program Enrolment Form
Please complete this enrolment form if you would like to be considered for one of our School Holiday Band workshops.  This is to help us create bands of similar experience, age and musical interest.  Please indicate all dates that you are available to take part.  If we have enough interest, we can add extra workshops.  Band in a Day is for musicians who have been playing for 1 year or more and are fairly competent.  Beginner Band is for new and less experienced musicians – no prior experience required.  

We will contact you to let you know whether your application has been successful, and to confirm the date of your Band Workshop by Monday 23 December.  Applications after this date will be dealt with as they are made.
