THIS FORM NEEDS TO BE AFFIRMED EVERY DAY FOR EVERY ATHLETE BY 9:00 PM THE EVENING BEFORE PARTICIPATION.  

If the waiver is NOT completed for your athlete before 9:00 pm, your athlete must check-in with their INSTRUCTOR prior to entering the facility.

IT MUST BE COMPLETED ONCE PER STUDENT  DAILY.

The GOOGLE FORM waiver serves as an ongoing acknowledgment and agreement to all terms associated with the:

BAC CENTER OF EXCELLENCE, LLC Fall waiver for "Swim School" at BAC.  

As such, a minor (17&Under) athlete may NOT sign off.  A legal parent or guardian must complete the GOOGLE FORM daily.
