Holiday Support Hamper Request Form
Dear All,

Due to the financial impact of COVID -19 on many families; Wendell Park and St Saviours Church will be working in partnership to supply Holiday Food Hampers and gift boxes for families and individuals who may be in need over the holiday period.

If you find yourself in this category and would like to request a hamper/gift box, please complete the form below.  All requests will be kept strictly confidential.
 
The hampers will be delivered to your home by volunteers from St Saviours Church. There will also be the option for you to collect the hamper from the church if you would prefer.

We are asking for all requests to be made by Tuesday the 8th December. Deliveries will be made between Wednesday 16th December and Saturday 19th December.  

THE DEADLINE FOR SUBMISSIONS TO THIS FORM IS 23:59 ON TUESDAY 15th DECEMBER 2020.
