Equality and Diversity Monitoring Form –Recruitment
As part of our commitment to ensuring football in Dorset is For All and following on from achieving the Equality Standard for Sport, Dorset FA are pleased to be working towards completing the Preliminary Level of the Standard. To support us in ensuring Dorset football is accessible to all we wish to understand who has applied during this recruitment process so would appreciate your time in completing a very short, voluntary survey.  The data collected will be analysed by the Dorset FA Equality Working Group to help us better understand who is currently involved in the game in Dorset and plan for the future of football in the county.  

To protect individual confidentiality the survey is anonymous - the online format means that we do not collect any identifying information such as your name, email address or IP address. Responses will be stored in a password protected electronic format, and deleted once the data analysis is complete.
