Although this form is optional, filling out whatever you feel is relevant helps me get to know your horse, pony, mule, or donkey; be best prepared for our first appointment, and adds you to my client database.

If your animal kicks, rears, strikes, yanks/snatches/stomps their feet away, or sets backs, are you willing to provide Dormosedan or a trainer? I do not work on kickers.  *
Are you able/willing to commit to a 4 - 6 week trim cycle? *
Does your equine stand well for their hoofcare provider visits? 

Please describe their tendencies. This will help me plan how to time my appointments and prepare for any special handling. 

First appointments are generally longer, with the long-term relationship between horse and HCP taking precedence over the first trim. The trim is also conservative this first visit until we know what your horse prefers. 
Is your animal currently sound?

What do we already know about what contributes to their soundness or lack thereof in regards to hoofcare? 
What services are you interested in? 
What is your equine's current diet?
What is your equine's current living arrangement? 
What is your animal's job? A riding or packing animal? Do you show? Trail ride? Are they a very cute lawn ornament? What is their activity level?
Do you use boots, casting, Hoof Armor, glue, or composites for riding on most surfaces?

To the best of your knowledge, does your horse have any of the following pathology? 
Has your animal ever been trimmed with an angle grinder, Dremel, or buffy? I use rasps, but will use a grinder if given the opportunity.

Payment is due at the time of service. Poquito Valley and Antelope Meadows residents pay $60/trim. All other locations in Yavapai County pay $65/trim. The Verde Valley is $70/trim. Outside of these areas (Phoenix and Flagstaff) is $75/trim. Drafts start at $85/trim. I do not offer discounts. 

Set-up trims on overdue, distorted, and long or pathological hooves are subject to a higher charge, with subsequent trims reverting back to the normal price.

Please understand that as a small business owner, I must charge a fee for no-shows, cancellations made within 24 hours of the appointment, and extra handling due to an animal's uncompliant or unsafe behavior. The full service charge must be paid even if the animal is unable to be fully trimmed due to my costs in time, travel, childcare, risk, fuel, tools, insurance, and education.  *
