Family In Transition Identification
If your family is experiencing a loss of housing, please complete this confidential form and a school social worker will follow up with you to provide support.
Your family may qualify as a Family in Transition under the Mckinney Vento act if you live in any of these situations: doubled up and staying with other people or couch hopping, staying in a hotel or motel due to loss of housing, staying in a shelter, staying in a tent, camper, vehicle, or other building not designed for living, or if your family is here temporarily or for migrant work.
If your family qualifies for Mckinney-Vento support, you may be eligible for benefits such as, the right for your child to stay in their school of origin when you move boundaries, busing to and from your current residence, free lunch, free preschool for age eligible children. Your School Social Worker will also be able to provide additional information for community based supports that may benefit your family.
