Parental Waiver of Claim:  By checking the box below, I agree to allow my child to participate and I fully understand that there is an inherent risk associated with participation in baseball tryouts, practices, camps, clinics and games.  I hereby agree to assume all risk of injury in such activities.  In assuming all risk of injury, I agree to hold harmless and waive any claim for damages against Los Tigres Baseball, The Dugout Baseball Academy, its hired coaches, directors, volunteers, including any and all owners and/or lessees of venues utilized for these activities. I fully understand that in the event of a medical emergency while visiting, participating in, or watching any event (including, but not limited to the aforementioned events which hold an associated inherent risk), EMS will be called to render assistance and I will be fully financially responsible for any expenses involved. *

By checking the box below, I also certify and confirm that I have read the preceding information and understand the risks associated with baseball tryouts, practices, camps, clinics, games, and any other related activities, and I certify I am voluntarily participating.
