This form is for parents of any junior player who trialed for Orange Thunder junior representative teams but was unsuccessful in being selected in the final team, to request and obtain feedback on their non-selection.

Please note this form is not a means to dispute any decisions made by our club, but instead a means for parents and players to obtain feedback on key skill areas or parts of their game they can look to improve on, which will benefit the player's chances of being selected in future teams.
