To save paper and time, the disclosure document for each of my classes this year will be digital.  Please read through each section carefully.  Parents/guardians AND students should review the disclosure in the next section.  After reviewing the policies for this class, parent/guardian and student are asked to provide a "digital signature" are saying they understand the policies as outlined.

If you have questions or would prefer a paper copy to sign, please let me know.  THIS FORM MUST BE SUBMITTED NO LATER THAN TUESDAY WEDNESDAY JANUARY 13!!
