Please complete this form to apply for the 2021-2022 school year. To complete your application, use must submit a $40 application fee. At the end of the form, you will have the option to receive an email invoice for a direct payment from your bank account, or to mail a check.

Enrollment contracts will be offered in the order that applications are received. We will contact you in February or March to let you know if we have a space for your child.
