Dear Sir/Madam,
COVID-19 vaccines are
effective and reliable tools against the COVID-19 pandemic. Nearly 65% of the
global population has received at least two vaccine doses. However, hesitancy towards
the COVID-19 vaccines has become a significant challenge in the global campaign
against the pandemic. Hence, our research team is conducting an Online-based
Global Survey to assess the public's perspective regarding the COVID-19
vaccination and identify factors associated with vaccine hesitancy. The
research group would like to use the questionnaire to test the hypothesis of
the relationship between psychological stress and and the status of hesitation
with COVID-19 vaccines. The information you provide could help policymakers
improve strategies effectively and help you make appropriate decisions in your
community and worldwide.
Informed consent
In this survey, you
will answer a questionnaire, which contains 25 questions and takes
approximately 10 minutes of your time. Your participation in this study is
voluntary. You are free to decline participation, skip any questions you do not
wish to answer, blank or withdraw from this study at any time. All the
responses to the survey will be recorded anonymously. The data collected will
be kept confidential; no other people except for project handlers will have
access to them. However, the results generated from the data will be
disseminated to the public so that other people can use them.
