As of May 28, 2021, VBS will not require volunteers nor children to wear masks. You may wear one if you wish.

WAIVER & PHOTO PERMISSION: I desire to participate in the Christ Pewaukee Vacation Bible School located on the property indicated. I acknowledges that there are dangers and risks of injury inherent in physical activities, but still participate in said activities. In addition, the undersigned gives the church staff and VBS volunteers permission to photograph me and to use those photos in print publications and websites (nametags with first names may be visible).  DISCLAIMER: Coronavirus, COVID-19 is a virus that spreads through person-to-person contact. COVID-19 can lead to severe illness, personal injury, permanent disability, and death. Participating in Christ Ev. Lutheran Church programs or accessing Christ Ev. Lutheran Church facilities could increase the risk of contracting COVID-19. Christ Ev. Lutheran Church in no way warrants that COVID-19 infection will not occur through participation in Christ Ev. Lutheran Church programs or accessing Christ Ev. Lutheran Church facilities. THEREFORE, the undersigned, for and in consideration of the opportunity to participate in VBS and for other good and valuable consideration, does hereby accept full responsibility for liability and cost of treatment of injury/illness to the name below. The undersigned does hereby release the teachers, assistants, property owners, pastors, and church staff from all liability resulting from negligence and agrees to hold them harmless for any injuries, illness, and/or damages I may sustain arising out of the activities inherent in a program of this type. The undersigned further declares and represents that no promise, inducement or agreement not herein expressed has been made to the undersigned, and that this release contains the entire agreement between the parties hereto, and that the terms of this release are contractual in nature and are not a mere recital. By entering my name below, I am stating that I have read and understood the above statement, and that I agree with its terms. I also understand that entering my name constitutes a legally-binding signature.
