2021 - 2022 IHSFA School Certification Form
Schools must be registered and payment must be postmarked by December 1, 2021, in order to be eligible for sweepstakes points and trophies at IHSFA post-season tournaments.

This form must be submitted online.  After registration is submitted, an invoice will be emailed to you immediately. **Please only complete the form once. If you need your invoice re-sent, please email Communications@ihsfa.org**

Registration is only considered complete upon receipt of payment.

Schools that fail to register and/or make payment are INELIGIBLE to participate in IHSFA post-season tournaments.

2021 - 2022 Annual Dues:
Postmarked by 12/1/2021 -- $115.00
After 12/1/2021 -- $140.00
After 1/11/2022 -- $165.00

Any fines imposed because of any check returned by the bank due to insufficient funds, closed bank accounts, or any additional fines/fees, are the responsibility of the school/school corporation submitting that check in addition to the original amount for which the check was written.

Submission of this form certifies that the named coaches are responsible for the entry of the school in the Sectional and State Tournaments. They are certified to enter your school's students for IHSFA tournaments. All entries submitted must be full time students in good standing at their high school. If the coach listed on this form will not be attending the tournament, the principals MUST authorize another individual and email it to the Executive Director. (All emails will be sent to the coach's school email unless another email is designated).
