Southeast Region 2019 Y.O.U.and Y.O.U. Sponsor Registration
This form is for: Youth Of Unity Members and Sponsors attending 2019 Regional Events.  FOR THE TEENS, THIS FORM IS TO BE FILLED OUT BY THE PARENT/GUARDIAN OF THE YOUer ATTENDING.

After filling out this registration form - you must download and fill out the medical form, which the Chapter Sponsors will send to the Medical Team, as well as bring a copy to the rally. It is suggested that an original is kept at the Church, too, in case it is forgotten at home.

YOUer or Sponsor information
YOU stands for Youth Of Unity - member of the high school youth group for the Unity Church. A Sponsor is an Adult Leader.
 If you have two YOUers from the same family, you would need to fill out two of these online forms.

In order to attend a YOU event, YOUer’s must meet 2 requirements: be in grades 9, 10, 11, 12 or equivalent during the academic calendar year, and must have a minimum age of 14 and a maximum age of 19. In order to attend a regional YOU event, YOUer’s must have attended at least 6 YOU Sunday lessons in the 3 months preceding registration, unless they are new. All participants must be able to honor heart agreements, function in a group setting and MUST attend the entire weekend from check-in thru Closing Ceremony. Youth with special challenges requiring close, personal adult supervision should be accompanied by a parent or familiar adult who will not be responsible for the rest of the group.
