This form will help us set up a payment plan that fits your needs. Please fill out the information below as accurate as possible. Thank you! 

We will set up the scheduled invoices via Square, which will be emailed to you, and you can pay online with a credit card. Typically course material access begins when you're half paid (though exceptions are sometimes made if a live stream series is about to start - we do like to ensure our out of pocket expenses are covered).

FYI if you are also applying for a scholarship discount, please can fill out the scholarship form for Home Herbalist Series or Beyond the HHS here or Advanced/Energetics here FIRST. THEN, fill out *this* payment plan form.
