Please complete the following form by Friday 21st May in order to be allocated a place at the Year 1 & 2 mixed Dance club. There are a maximum of 20 places available, allocated at random once the sign up date has passed. Once all places have been filled, confirmation of a place along with club dates and payment information will be sent via email. The total cost for all sessions will be £10.00.

*Please ensure that costs for all other services (Breakfast & Teatime Club, School Meals, other Sports Clubs etc) have been settled before completing this form. Unsettled accounts will not be considered for new services.
