This form will be taken as an official entry into the Masters Nationals Team Competition 2024. Please submit an official entry form for each team you are entering.

Entries must be received by 20th September 2024.

All teams shall consist of six male players and six female players who must cover the following age categories:
35-39 age group One male, one female
40-44 age group One male, one female
45-49 age group One male, one female
50-54 age group One male, one female
55-59 age group One male, one female
60+ age group One male, one female 

Please complete all details of all players below (including reserves). Indicate each player's actual age group they will play in the individuals.

All players entered will be also be entered in the individual draw unless otherwise advised.
