Garden Spot Athletics Covid-19 Pre-Screening Form for Voluntary Off-Season Workouts
This form will be available as of July 1, 2020 and must be completed within 30 minutes of participating in each off-season workout.
Please Note:
1) All questions are required. Failure to answer every question will prevent submission of the form and will result in the student-athlete or coach being prohibited from participation.
2) If any responses are “YES” or symptoms are reported, the student-athlete/coach will NOT be allowed to participate, must leave school grounds, and a Parent/Guardian will be notified. This is in compliance with CDC, PDE, & PIAA Guidelines.
