I agree that my child will not attend any class if they have any symptoms of illness. *
I consent to my child (or ward) attending art classes organized by the Boissevain-Morton Arts Council, and hereby release and discharge the Arts Council and its volunteers, affiliates or employees, as well as the class facilities, for any injury to person or property during participation therein. In the case of medical emergency, I understand that every effort will be made to contact the parent/guardian contacts listed, but in the event contact can not be made or is delayed I authorize the Arts Hall staff to secure necessary medical treatment for my child (or ward) during the event. I understand personal contact information may be used by the Arts Council to contact individuals involved for future Art Hall events *
