Thank you for allowing our School Nutrition Team to serve your family during this time of quarantine.  The meals will include both breakfast, lunch, supper and snack meals to cover a student's nutrition for 5 full days.

Please know we will do a home delivery as soon as we are able from the time this form is completed by the parent/guardian.  Please fill out a form for EVERY CHILD IN THE HOUSEHOLD that qualifies for virtual learning as requested from the COVID hotline team.  

We will contact you for delivery specifics via Google text or email once we receive this completed form.

Meals will include a full frozen pizza, easy to microwave or air fry entrees, fresh, frozen and canned fruits and vegetables along with low fat milk.  Please note, the snack is built into the fruits and vegetables in individual cups or bags. Our goal is nutrition and convenience for students working and being educated from home during this time.
