(referred to as the "Class") offered by Amplio Fitness LLC involves a risk of physical injury. I understand that any physical activity, including exercise, has inherent risks and I voluntarily choose to participate in the Class with full knowledge of the risks involved.
I represent and warrant that I am physically fit and have no medical condition that would prevent me from participating in the Class. I acknowledge that it is my responsibility to consult with a physician prior to and regarding my participation in the Class.
I agree to follow all instructions and recommendations given by the instructor and to discontinue participation if I experience any pain or discomfort.
In consideration of being permitted to participate in the Class, I hereby release and discharge Amplio Fitness LLC, its owners, agents, employees and volunteers from any and all liability, claims, demands, actions and causes of action whatsoever arising out of or related to any loss, damage, or injury, including death, that may be sustained by me, or to any property belonging to me, as a result of my participation in the Class.
By signing my name at the bottom of this form, I agree to indemnify and hold harmless Amplio Fitness LLC from any and all claims or lawsuits brought by myself or any third party as a result of my participation in the Class. *
