COVID EMERGENCY FUND REQUEST
In response to the COVID-19 crisis, SUNY Upstate converted most classes to online learning during the Spring, Summer and Fall 2020 semesters as well as canceled in-person clinical experiences. The Federal Coronavirus Aid, Relief, and Economic Security (CARES) Act authorizes colleges to award emergency grants to students for expenses related to the disruption of campus operations due to the coronavirus, such as food, housing, course materials, technology, health care, and child-care expenses. This form allows students to request a Federal CARES, Emergency Grant.
Complete the request below if you, the student, have incurred expenses that are related to the disruption of campus operations due to coronavirus. For an incurred expense to be eligible, it must be related to the disruption of campus operations. Eligible expenses include food, housing, course materials, technology and computer, health care, dependent care, transportation, and other. Please note, this fund is ONLY for COVID Related expenses and can NOT be used towards tuition or existing outstanding bills.
