This form is for monthly tuition autopay only, does not apply to drop-ins, punchcards, camps or intensives. 

Autopay typically runs September-June

The card on file in your MindBody account will be charged at the first of each month September-June, or the first month you have initiated autopay through June.  

*** Form must be submitted by the 25th of the prior month to end or adjust autopay for the following month(s), no refunds will be given for failure to drop or change autopay tuition past this date and charges will go ahead as scheduled.
