CVHS ASB Activity / Fundraising Request Form
Please make sure you fill out this form completely and submit it at least two weeks prior to the Monday preceding your event.  Forms not turned in according to this time frame will most likely be denied.  Upon submission the club advisor will be contacted to confirm the event.  

CVAC reviews all requests on Monday afternoon and will inform the staff advisor of approval status.  

Please fill out an ASB PO form if expenses will be incurred in conjunction with this event.

A Revenue Recap (available on the clubs website) must be completed when the project is complete.
