Dear Parents and Guardians:  The Manchester School District provides students access to many effective web-based learning tools. In order for our students to use these programs and services, certain personal identifying information -- generally the student’s name and email address -- must be provided to the operator of the website or application. In order to assist these operators to comply with the Children’s Online Privacy Protection Act (COPPA), theManchester School District is providing this notice and consent form seeking parental permission for the creation of web-based student accounts for all children under age 13.    COPPA allows schools to consent to the collection of limited personal information on behalf of students, but we only do so when parents or guardians have given permission.Commonly used web-based learning tools include G Suite for Education (formerly Google Apps forEducation), i-Ready, and EverFi. A list of district approved websites and applications can be found attrc.mansd.org/coppa. This list includes links to each website or application’s privacy policy, which is a document that describes what information the website or application collects about students, how it uses that information, who it shares it with and how it protects it. We recommend that you review these policies before signing this form. Please note that COPPA does not apply to general internet use that does not require a student account.The Manchester School District does not endorse any services that collect a student’s physical address, telephone number or social security number. Services that allow additional third-party sites to directly contact or market to students in the Manchester School District are not endorsed.This form constitutes your consent for Manchester School District to provide limited personal information for your student consisting of name and username, email address, and in limited cases, date of birth for confirmation of age. This permission will be recorded in the district student information system and will remain in effect until revoked by the parent(s) or guardian(s). Your child will not be allowed to use any of the websites or applications listed at trc.mansd.org/coppa, and consequently those sites and apps will not collect, use, or disclose any information about your child, unless and until you consent to such use by signing and returning this form. To provide your consent please complete this form and mail, fax, or scan and email it to your child’s school.All technology use in Manchester Schools is monitored and subject to the Responsible Use of Technology inSchools policy. *
