IF YOU want your student to participate in the full time in-person learning model, you MUST complete this form. Please complete one form per student. **Prior to completing this form, you MUST carefully read the "Parent and Student Guidelines." The guidelines may be found on the school's website at cjkellerelementary.ccsd.net Parents must complete this form in order for their student to participate in full time in-person learning. If you need assistance or have any questions, please call the school office at 702-799-2140 between the hours of 7:30 a.m. and 4:00 p.m. Monday through Friday. The first day of the full time in-person learning is Tuesday, April 6, 2021.
