Scratch Form for all Individual events. Except for 800/1500 distance events, which require positive check-in to swim, an athlete is assumed to be swimming unless scratched.  
Only ONE athlete may be scratched per submission.
Email is collected for receipt of form. Please use coach in attendance email.
ALL PRELIM SCRATCHES ARE TIME STAMPED.  DO NOT USE THIS FORM POST SCRATCH DEADLINE- YOU MUST DFS WITH ADMIN.  SCRATCHES FROM FINALS MUST BE DONE AT ADMIN.
